Sports Injury (Player Accident) Insurance

This page explains how to report an injury and make a claim under AFDA’s Sports Injury (Player Accident) insurance.

This process applies only to participant injury claims.

It does not relate to Public Liability or Professional Indemnity insurance.

Insurance eligibility and registration 

AFDA’s Sports Injury insurance is event-based and applies only to eligible participants who are properly registered at the time of the incident.

As part of the claims process, AFDA and/or the insurer may need to verify that the injured participant was registered in an AFDA-approved registration system for the relevant event or activity.

Approved registration systems:

  • Ultimate Central – AFDA’s primary national registration system
  • GameDay – approved only where formally authorised by AFDA (currently Western Australia as a trial state)

If you were registered in an AFDA-approved system and were injured while participating in an official flying disc activity conducted under AFDA authority, you may be eligible to make a claim.

What to do if an injury occurs

If an injury occurs during an activity covered by AFDA’s Sports Injury insurance, the following steps must be followed.

  1. Seek medical advice appropriate to the injury.
  2. Notify a relevant official as soon as practicable, such as:
    • a Club Official
    • a State Association representative
    • an AFDA representative (for AFDA-run events such as national championships or training camps)
  3. Complete the AFDA Injury Report Form (mandatory).
  4. Complete the Sports Injury Claim Form (mandatory).

The AFDA Injury Report Form must be completed to verify eligibility and registration details before a claim can be lodged.

How to submit

Once all required sections are completed, submit the forms and any supporting documentation to:

AFDA will review the information and coordinate lodgement with the insurer.
